Okay. There are several reasons for why you would be looking at this screen.
In all probability you either forgot to fill in a required field for the form.
If the error is something you overlooked or mistyped, use the back button to return to the form, fix the problem and try again.
Or, perhaps the PERL scripts that are running this form went out to lunch and left food on the table attracting a bunch of pesky bugs.
